I have a question. If I buy an Extended version of this plugin, will I be able to use it for more than one website server? I don’t clearly uderstand the term “up to 5 sites”. It means 5 servers (e.g. http://www.xx1.yy, http://www.abc.zz etc) or 5 pages in one server (e.g. http://www.xx1.yy/id=1, http://www.xx1.yy/id=2 etc.)?

Hi,
The number of sites refers to the number of WordPress sites you run. It does not refer to the number of pages nor domains. If you have 10 WordPress installed sites on a single server and if you purchase the Extended “Up-to-5” license type, 5 of them will be able to activate the license and receive automatic updates.

The form I have on the top right column of the home page http://www.lawyersgroup.com/wp/ seems like it is being blocked, not working, by the responsive column text widget it is inside. The same code for the form is being used on the child pages which is a text widget inside the primary sidebar and is working properly. I am using the most up to date version. Can you advise on how to fix?

Are you sure that the form with the same code in the child pages really does work?
As I see the HTML source code of your site, the form tag is not enclosed properly. The input tag elements are outside the form tag now. It is also happening in the child pages as well.
